These Mortar and Pestle Shaped Name Tags are perfect for any medical profession. Show your patients how much you care by customizing these professional name tags with your logo, business colors and text.

PROOF: You will receive a digital proof of your name tag for you to approve. We maintain your unique design for easy reordering.
SET-UP FEE: Blank and text only tags charge $10.00 for the custom shape; $25.00 is charged for custom shape tags with a logo. For reorders of the same layout, no set-up fee is charged.
Custom shapes come in a plastic, two-ply engravable material with a 0.06 inch thickness. The materials have either a brushed, smooth, gloss or matte finish depending on your color choice. Orders are completed within one to four business days after a proof approval has been received.
TEXT AND LOGO: Tags come with engraved text and logos. Or, order them blank and add labels.
LIST ENTRY: Lists must be properly formatted and include all of the information for your name tags. If a list is required it can be entered or uploaded from this page, which will allow for faster processing time. Or, the list may be emailed to nametag@nametag.com and will need to include your contact information as placed with this order.
ADDITIONAL INFORMATION ABOUT LIST SUBMISSION (opens in a new tab)DOME: Adding a dome finish to your name tags will provide a slightly raised, scratch resistant surface. If you are ordering a blank shape, we do not recommend a dome as adding names may damage the tag.
