PROOF: Custom designed name tags come with a digital proof which must be approved to send your order into production. This layout is maintained to make reordering easier.
SET-UP FEE: Text only or blank are $10, engraved logo is $25, reorders of the same layout do not have a set-up fee.
Use the order form to choose whether you want your name tags to have text, a logo, both or to be left blank. These name tags are laser cut from 0.06 inch, two-ply plastic that is perfect for custom engraving.
LIST ENTRY: Lists must be properly formatted and include all of the information for your name tags. If a list is required it can be entered or uploaded from this page, which will allow for faster processing time. Or, the list may be emailed to nametag@nametag.com and will need to include your contact information as placed with this order.

FASTENERS: Custom shaped name tags come with a variety of fastener choices with no extra charge or deluxe magnets for $1.10 per tag.
SURFACE: Depending on the color combination you select, these tags will have a matte, smooth, glossy or brushed surface.
DOME FINISH: These tags can have a protective dome finish for $3.50 per tag.
PRICE INCLUDES: Name Tag with Notched Corners with one line of engraving, a standard fastener (pin, bulldog clip, bulldog clip with pin, double clutch).
SET-UP FEE: $10.00 for blank and text only tags, $25.00 for tags with a logo. No set-up fee is charged on reorders of the same layout.
SHIPPING: You will be given the opportunity to select a shipping method during checkout.
