PROOF: A digital proof of your design will be sent for your approval. We save your custom layout so that reorders are processed quickly.
SET-UP FEE: $10.00 for blank and text only tags and $25.00 for tags with a logo. You are not charged a set-up fee on reorders for the same layout.
Two-ply name tags in an engravable plastic material are available in 30 color combinations. Order as text only, with a logo, logo and text or blank. Customize the options to make the perfect name tag for your business.
LIST ENTRY: Lists must be properly formatted and include all of the information for your name tags. If a list is required it can be entered or uploaded from this page, which will allow for faster processing time. Or, the list may be emailed to nametag@nametag.com and will need to include your contact information as placed with this order.

ARTWORK REQUIREMENTS: To add a logo to this name tag, upload it on the order form in a vector art (.eps, .ai, .cdr) format.

PRICE INCLUDES: Fish Shaped Name Tag with one line of engraving and a standard fastener (pin, bulldog clip, bulldog clip with pin, double clutch).
SET-UP FEE: $10.00 for blank and text only tags, $25.00 for tags with a logo. No set-up fee is charged on reorders of the same layout.
