All personalized products require a list (Classic Name Tags, Photo ID's, Classic Name Plates, Modern Name Plates and Executive Name Plates). If a list was required on the initial order, a list will be required for all reorders.

has been received by Name Tag, Inc.
Note - Lists entered on this form will only have one list saved with this order. If you make any changes to your list and save them, only the last saved list will be forwarded for production.
Enter text EXACTLY as you want it to appear on the name tags (including upper/lower case letters).
Long lines of text may be condensed to fit the available space.
The quantity of product ordered will be taken directly from your list. Lists must contain all text to be printed - for duplicate tags enter the text twice.
Proof lists carefully: your list will be used to print your tags, Name Tag, Inc. is not responsible for lists that are formatted incorrectly or contain errors.
Uploaded lists must be submitted in one of these formats--.doc,.xls,.txt,.odt,.sxw,.xls,.odm,.sxc,.ods,.pdf,.wps,.wpd,.rtf.
Name Tag, Inc. will not edit your list. Lists not prepared to our specification will be returned for correction, which may cause a delay in production.
Company policy requires that all lists be in writing. Names (even just one name) will not be accepted over the phone. Lists can be presented in the form of an attachment in an email, on a floppy disk, or on a CD. Faxed lists are acceptable, but a data entry fee will be charged to type the list into our system.

KEEP SAME PAYMENT INFORMATION: We will use the information provided on your last order to process this order.
UPDATE PAYMENT INFORMATION: You will be contacted for new payment information before your order is sent to production.
Production - Completed orders (with a list) must be received by 5:00 Mountain Time, or they will be processed the next day. If the production time and/or the shipping method need to be adjusted to meet your 'need by date' you will be contacted before production begins.
EXPEDITE FEE: $50, Expedited production may not always be available. You will be contacted if meeting your 'Need By' date requires additional production or shipping charges.
ADDITIONAL FEES: $3 handling fee plus the cost of shipping. There is no minimum quantity on reorders of an existing layout (design).
Shipping - All packages will be shipped by UPS (except military addresses which will ship USPS Priority Mail. If the shipping method (Ground, 2 Day Air, Next Day Air, etc.) needs to be adjusted to meet your 'need by date' you will be contacted before production begins.
