IMPORTANT INSTRUCTIONS:
Enter text EXACTLY as you want it to appear on the name tags (including UPPER/lower case letters).
Proof lists carefully: your list will be used to engrave your tags, Name Tag, Inc. is not responsible for any consequences of lists formatted incorrectly or with errors.
Name Tag, Inc. will not edit your list. Lists not prepared to our specification may be returned for correction, which may cause a delay in production.
Company policy is to require that all lists be in writing. Names (even just one name) will not be accepted over the phone. Lists can be presented in the form of an attachment in an email, on a floppy disk, or on a CD. Faxed lists are acceptable but a data entry fee will be charged to type the list.
Lines of text that are constants (do not change from tag to tag and order to order) do not need to be included in your list. These lines of text may be entered once at the top of your list and labeled appropriately. For pre-designed tag styles all lines are considered variables.

Information for each line of text must be on a separate line, starting at the top of the tag. A blank line should be left between the information for each tag. Multiple blank lines may be left between the information for each tag, if they are consistent through the list If you are requesting duplicate tags enter the information for those tags multiple times. Remove all formatting from your list. A line of descriptive text may be inserted into a list to describe a unique feature of a group of tags, (Type of fastener, layout # # of lines, etc.) Remember, your list will be used exactly as submitted please verify all spelling and punctuation. |

| LIST SUBMISSION IN A SPREADSHEET FORMAT |
Remove all formatting and filtering from your spreadsheet. If you have any question about the formatting, copy and paste your columns into Notepad. If you see any difference in spelling or special characters ( ń, é ®, etc.) your spreadsheet has been formatted or filters have been applied and the spreadsheet is unacceptable for our use. Each line on your tag needs to be entered into its own column with all the information for a tag entered onto a single row. If you are requesting duplicate tags enter the information for those tags the required number of times A column of descriptive text may be inserted into a list to describe a unique feature of a group of tags (type of fastener, layout # of lines etc.) Remember, your list will be used exactly as submitted please verify all spelling and punctuation.

A line of descriptive text may be inserted into a list to describe a unique feature of a group of tags, (Type of fastener, layout # # of lines, etc.) Your list may also be submitted using a spread sheet shown above, include the exact name of the photo in one column of your spreadsheet.
