A custom layout will be created to your specifications and a paper proof provided prior to production. The top layer of two-ply .06 inch thick plastic is removed by the laser engraver, showing the second color and creating a two color plastic name plate. These name plates are text only - if you would like a logo please visit the Engraved Logo Order Form.

PRICES START AT: 5 name plates for $5.41 each, 100 for $3.63 each. Discounts are available for other quantities.

Custom sized name plates are also available

Custom sized name plates are limited by the size of the plastic, the engraving machines and the square inch size.

MINIMUM SIZE: 12 square inches. | MAXIMUM SIZE: 24 square inches.

Larger plates may be ordered on the Sign pages.

Smaller plates may be ordered on the Name Tag pages.

PLASTIC SIZE: 12 inches x 24 inches.

MINIMUM WIDTH OR HEIGHT: 0.75 inches.

This form is set up to calculate the price of the nameplates, based on the information entered. To do these calculations yourself use the following formulas.

CUSTOM SIZED NAME PLATES = Price for 2x8 name plate + ((sq inch-16) * price per sq inch).

No color guarantee is made by these images, if a color match is required please request a sample.

Adhesives can be applied directly to the back of the name plate or can be used in conjunction with a name plate holder (example: applied to the back of a wall mount).

You will receive a paper proof via email, showing a representation of your plate layout.

Follow these specs -- Using the specs you outline on this order form we'll create a design for your plates and send you a paper proof for your approval.

Fonts

To use a font that is not listed select 'Custom Font', in the drop down, enter the name of the font you would like to use. If your selected font is not on our system, you may provide us with a PC format font file, or we can select a similar font.

Emphasize a company name by choosing a font that is different from the names and titles.

Design Requested -- Our team of designers will set-up your name plates, creating a custom layout for your design.

Customer supplied layout -- We cannot engrave directly from your file, but will follow your specifications as closely as possible, a paper proof showing the final plate design will be emailed to you for approval.

Every line of text on a name plate is counted as a separate text line.

Enter text EXACTLY as you want it to appear on the name plates (including upper/lower case letters). Note: Our computer software is automated to remove additional spacing between words, so any extra spaces entered with your text will not be included.

Long lines of text may be condensed to fit the available space.

Lists must contain all text to be engraved - for duplicate plates enter the text twice

Proof lists carefully: your list will be used to engrave your plates, Name Tag, Inc. is not responsible for lists that are formatted incorrectly or contain errors.

Name Tag, Inc. will not edit your list. Lists not prepared to our specification will be returned for correction, which may cause a delay in production.

Company policy requires that all lists be in writing. Names (even just one name) will not be accepted over the phone. Lists can be presented in the form of an attachment in an email or on a CD. Faxed lists are acceptable, but a data entry fee will be charged to type the list into our system.

Proof - After your order is placed, artwork is received, and a properly formatted list of names has been received, a digital proof will be created and emailed to you within 4 business hours -- If changes to the paper proof are requested a new proof will be provided. Production WILL NOT begin until an approved, signed proof is received by Name Tag, Inc.

Production - Once an approved proof has been received, production begins. If the production time and/or the shipping method need to be adjusted to meet your 'need by date' you will be contacted before production begins.

Completed orders (with a signed proof, and a properly formatted list) must be received by 5:30pm Mountain Time, or they will be processed the next day.
